ABSTRACT

SUMMARY: The spleen is the largest intraperitoneal lymphatic organ of the body, presenting two main functions: defensive, through immune response and blood filtration. The objective of the present review was to obtain updated information on the anatomy of the spleen of the albino rat (Rattus norvegicus albinus) and to compare it with the anatomy of the human, dog, cat and pig spleen, representing the main species of importance in medicine, veterinary medicine and biomedical sciences. A search for updated bibliographic material was carried out in different scientific websites. Thus, 71 bibliographic sources were reviewed, mostly scientific articles (31), human and veterinary anatomy books (17), specialized articles (17) and theses (6). In general, there is consensus on the anatomical description of the spleen, which is located in the left hypochondriac region of the abdomen between the fundus of the stomach and the diaphragm, irrigated by the splenic artery and vein. It was evidenced that there are similarities in macroscopic aspects when comparing the spleen of the white rat with the spleen of other species (functionality, relative weight, topographic location). In microscopic aspects, the spleen in humans and other mammals is composed of stroma, in addition to parenchyma, constituted in turn by white and red pulp. In particular, there are differences between rat, human, cat, dog and pig spleens in shapes, sizes and microscopic aspects related to microcirculation and immunity. While there are similarities in pathological processes and responses to pharmacological and clinical treatments. For the above mentioned, it is concluded that the albino rat constitutes a good biological model, specifically in microscopic anatomical aspects of the spleen of immunological type. While the pig spleen is comparatively better in macroscopic anatomical studies of surgical type, both are extrapolable especially to human medicine.

ABSTRACT

Hippocrates was one of the greatest physicians ever, and merited unquestionably, the title of 'Father of Medicine'. He, together with other authors, formed an assemble of texts known as 'Hippocratic Collection' (or 'Hippocratic Corpus'). There, the ample and brilliant medical experience remained documented, and left as a legacy. Even after almost two and a half millennia, these medical writings continue to be admired, and regarded as a lasting fount of inspiration. However, regarding anatomy, including that of the nervous system, and related structures, the texts are poor. About the nervous system, he mentioned the brain and its two hemispheres, the spinal cord, and the sensory organs. The related structures are also cited, comprising the meninges, the cerebrospinal fluid, and the blood vessels, as well as the skull, and the vertebral column. The functions of the brain were partly explained, and he ascribed to the brain the main ruling function of the body, a mark of encephalocentric position. Thus, counterposed to his ample medical accomplishments was his poor anatomical contribution. However, it must be recognized that the incipient contribution on anatomy that was legated, represented an important starting point for upcoming scholars and further studies on the subject.

ABSTRACT

The human sensory receptors are morphologically specialized to transduce specific stimuli into the brain. However, when an injury occurs, mainly in the spinal cord, which can be of traumatic or non-traumatic origin, it provokes various degrees of sensory deficits, autonomic, motor and sphincter dysfunction below the level of the injury. Based on this, a new therapeutic modality is being proposed by neuroscientist Miguel Nicolelis, which is based on the brain-machine interface, that is, using other pathways so that the information can reach the cerebral cortex and thus be consciously processed (AU).

ABSTRACT

Aristoteles was probably the first anatomist in the modern sense of this term. He wrote on human anatomy relying on the external aspects, and the lacking data on internal structures were generalized through the study of lower animals. The different body regions were described according to a precise topography, and his contribution to the development of anatomy was extensive, as he correctly described many organs and introduced new terms to indicate anatomical structures. Regarding the nervous system, he identified the brain and the two hemispheres, the cerebellum, and the spinal cord. The brain coverings, the meninges, were identified, a tougher external, and a delicate internal, and the related blood vessels. He described the bony casing, the skull and part of its bones and sutures, as well as the vertebral column formed by distinct holed vertebrae. The sensory organs were also defined, but he overlooked the presence of nerves, confusing such structures with vessels, tendons, ducts, among other similar structures. Additionally, he explained the functions of the brain, to which he attributed an important role, despite his cardiocentric standpoint. More than twenty and three centuries have elapsed since Aristoteles began his biological investigation, and his work was and continues to be admired, despite the inaccuracies that were pointed out by later authors. Evidently, knowledge on anatomy of the nervous system before Aristoteles was very scant, thus, it must be recognized that the pioneer anatomical studies he performed may be seen as fundamental, leaving a solid ground for future research on anatomy.

ABSTRACT

Objective:To study the improvement of normal tissue region of interest (ROI) segmentation based on clustering-based multi-Atlas segmentation method, thereby achieving better delineation of organs at risk. Methods:CT images of 100 patients with cervical cancer who had completed treatment in Zhejiang Cancer Hospital during 2019-2020 were selected as the Atlas database. According to the volume characteristic parameters of the organs at risk (bladder, rectum and outer contour), the Atlas database was divided into several subsets by k-means clustering algorithm. The image to be segmented was matched to the corresponding Atlas library for multi-Atlas segmentation. The dice similarity coefficient (DSC) was used to evaluate the segmentation results. Results:Using 30 patients as the test set, the sub-Atlas generated by different clustering methods were compared for the improvement of image segmentation results. Compared with general multi-Atlas segmentation methods, clustering-based multi-Atlas segmentation method significantly improve the segmentation accuracy for the bladder (DSC=0.83±0.09 vs. 0.69±0.15, P<0.001) and the rectum (0.7±0.07 vs. 0.56±0.16, P<0.001), but no statistical significance was observed for left and right femoral head (0.92±0.04, 0.91±0.02) and bone marrow (0.91±0.06). The average segmentation time of clustering-based multi-Atlas segmentation method was shorter than that of the general multi-Atlas segmentation method (2.7 min vs. 6.3 min). Conclusion:The clustering-based multi-Atlas segmentation method can not only reduce the number of Atlas images registered with the image to be segmented, but also can be expected to improve the segmentation effect and obtain higher accuracy.

ABSTRACT

Objective:To develop a multi-scale fusion and attention mechanism based image automatic segmentation method of organs at risk (OAR) from head and neck carcinoma radiotherapy. Methods:We proposed a new OAR segmentation method for medical images of heads and necks based on the U-Net convolution neural network. Spatial and channel squeeze excitation (csSE) attention block were combined with the U-Net, aiming to enhance the feature expression ability. We also proposed a multi-scale block in the U-Net encoding stage to supplement characteristic information. Dice similarity coefficient (DSC) and 95% Hausdorff distance (HD) were used as evaluation criteria for deep learning performance. Results:The segmentation of 22 OAR in the head and neck was performed according to the medical image computing computer assisted intervention (MICCAI) StructSeg2019 dataset. The proposed method improved the average segmentation accuracy by 3%-6% compared with existing methods. The average DSC in the segmentation of 22 OAR in the head and neck was 78.90% and the average 95%HD was 6.23 mm.Conclusion:Automatic segmentation of OAR from the head and neck CT using multi-scale fusion and attention mechanism achieves high segmentation accuracy, which is promising for enhancing the accuracy and efficiency of radiotherapy in clinical practice.

ABSTRACT

Objective:To explore the method of constructing automatic delineation model for clinical target volume (CTV) and partially organs at risk (OAR) of postoperative radiotherapy for prostate cancer based on convolutional neural network, aiming to improve the clinical work efficiency and the unity of target area delineation. Methods:Postoperative CT data of 117 prostate cancer patients manually delineated by one experienced clinician were retrospectively analyzed. A multi-class auto-delineation model was designed based on 3D UNet. Dice similarity coefficient (DSC), 95% Hausdorf distance (95%HD), and average surface distance (ASD) were used to evaluate the segmentation ability of the model. In addition, the segmentation results in the test set were evaluated by two senior physicians. And the CT data of 78 patients treated by other physicians were also collected for external validation of the model. The automatic segmentation of these 78 patients by CTV-UNet model was also evaluated by two physicians. Results:The mean DSC for tumor bed area (CTV1), pelvic lymph node drainage area (CTV2), bladder and rectum of CVT-UNet auto-segmentation model in the test set were 0.74, 0.82, 0.94 and 0.79, respectively. Both physicians' scoring results of the test set and the external validation showed more consensus on the delineation of CTV2 and OAR. However, the consensus of CTV1 delineation was less. Conclusions:The automatic delineation model based on convolutional neural network is feasible for CTV and related OAR of postoperative radiotherapy for prostate cancer. The automatic segmentation ability of tumor bed area still needs to be improved.

ABSTRACT

Objective:To compare the effect of neoadjuvant chemotherapy vs. concurrent chemoradiotherapy on the target volume and organs at risk for locally advanced bulky (>4 cm) cervical cancer. Methods:From March 1, 2019 to June 30, 2021, 146 patients pathologically diagnosed with cervical cancer were selected and randomly divided into two groups using random number table method: the neoadjuvant chemotherapy (NACT) + concurrent chemoradiotherapy (CCRT) group ( n=73) and CCRT group ( n=73). Patients in the NACT+CCRT group received 2 cycles of paclitaxel combined with cisplatin NACT, followed by CCRT, the chemotherapy regimen was the same as NACT. In the CCRT group, CCRT was given. Statistical description of categorical data was expressed by rate. The measurement data between two groups were compared by Wilcoxon rank-sum test for comparison of two independent samples, and the rate or composition ratio of two groups was compared by χ2 test. Results:Before radiotherapy, GTV in the NACT+CCRT group was (31.95±25.96) cm 3, significantly lower than (71.54±33.59) cm 3 in the CCRT group ( P<0.01). Besides, CTV and PTV in the NACT+CCRT group were also significantly lower compared with those in the CCRT group (both P<0.05). In terms of target volume dosimetry, D 100GTV, D 95CTV, V 100GTV, V 100CTV and V 95PTV in the NACT+CCRT group were significantly higher than those in the CCRT group (all P<0.05). The complete remision (CR) rates in the NACT+CCRT and CCRT groups were 86.3% and 67.6%, with statistical significance between two groups ( P<0.01) . Regarding organs at risk, NACT+CCRT group significantly reduced the dose to the bladder, rectum, small intestine and urethra compared with CCRT group (all P<0.05). Conclusions:NACT can reduce the volume of tumors in patients with large cervical masses, increase the radiation dose to tumors, reduce the dose to organs at risk, and make the three-dimensional brachytherapy easier. Therefore, NACT combined with CCRT may be a new choice for patients with locally advanced cervical cancer with large masses.

ABSTRACT

Objective:To compare the effect of uPWS R15 software based on deep learning with MIM-Maestro 6.9 software based on atlas library to automatically delineate the organs at risk of prostate cancer in order to provide a reference for clinical application. Methods:The CT data of 90 prostate cancer patients admitted to the Department of Oncology Radiotherapy of the Affiliated Hospital of North Sichuan Medical College from 2018 to 2022 were retrospectively selected. Based on the uPWS R15 software developed by Shanghai United Imaging Medical Technology Company and the MIM-Maestro 6.9 software developed by Beijing Mingwei Vision Medical Software Company, the effects of uPWS and MIM software on automatic delineation of organs at risk were evaluated according to five parameters, including delineation time (T), Dice similarity coefficient (DSC), Jaccard similarity coefficient (JSC), Hausdorff distance (HD) and the mean distance to agreement (MDA).Results:The sketching time of uPWS software was less than that of MIM software. There were no significant differences in the sketching effect of femoral head and skin between the two software (all P>0.05). The delineation of right kidney ( tMDA=-3.43, zDSC=-4.03, zJSC=-4.16, P<0.05), left kidney ( tMDA=-3.87, zDSC=-4.18, zJSC=-4.41, P<0.05), small intestine ( tMDA=-8.57, zDSC=-9.99, tJSC=14.21, P<0.05) and rectum ( zMDA=-4.00, tDSC=-9.98, tJSC= 9.72, P< 0.05) except HD, was statistically different. The bladder ( z=-7.88, -9.00, -8.17, -8.74, P<0.05) and spinalcord ( z=-3.87, -4.43, 4.03, 3.05, P<0.05) were also delineated with significant differences. The DSC automatically delineated by uPWS software was >0.7, while the DSC automatically delineated by MIM software was >0.7 for all other organs at risk except small intestine and rectum. In addition, the HD, MDA and JSC values of the organs at risk (bilateral femoral head, bilateral kidneys, spinal cord, bladder, skin, rectum and small intestine) automatically delineated by uPWS software were generally better than those with MIM software. Conclusions:The uPWS software outlines better than the MIM software, but the MIM software can also be used clinically with modifications to the small bowel and rectum, saving a great deal of time in preparation for radiation therapy.

ABSTRACT

Medication during pregnancy is widespread, but there are few reports on its fetal safety. Recent studies suggest that medication during pregnancy can affect fetal morphological and functional development through multiple pathways, multiple organs, and multiple targets. Its mechanisms involve direct ways such as oxidative stress, epigenetic modification, and metabolic activation, and it may also be indirectly caused by placental dysfunction. Further studies have found that medication during pregnancy may also indirectly lead to multi-organ developmental programming, functional homeostasis changes, and susceptibility to related diseases in offspring by inducing fetal intrauterine exposure to too high or too low levels of maternal-derived glucocorticoids. The organ developmental toxicity and programming alterations caused by medication during pregnancy may also have gender differences and multi-generational genetic effects mediated by abnormal epigenetic modification. Combined with the latest research results of our laboratory, this paper reviews the latest research progress on the developmental toxicity and functional programming alterations of multiple organs in offspring induced by medication during pregnancy, which can provide a theoretical and experimental basis for rational medication during pregnancy and effective prevention and treatment of drug-related multiple fetal-originated diseases.

ABSTRACT

Siddha system is the foremost traditional system, which was introduced by ancient Siddhar’s. According to Siddhar’s, nature is man, man is nature. Man is said to be microcosm and universe is macrocosm. The universe is made up of five elements namely Earth, Water, Fire, Air, and Space. Our body is also made up of five elements (Panchaboothas), three vital humors known as Vatham (air, space), Pitham (fire), Kabam (Earth, water), seven physical constituents are Saram (nourishing fluid), Senneer (blood), Oonn (muscles), Kozhuppu (adipose tissue), Enbu (bone), Moolai (bone marrow), Sukkilam/Suronitham (sperm/ovum) respectively. The universe has stars and planets. Each planet has an influence that reflects in our bodies. Thoughts were controlled by cosmic force in the universe. Nine planets in the universe correlate with the organs in the body Sun-Heart, Moon-Brain, Mercury-Lungs, Jupiter-Liver, Saturn-Spleen, Mars-Gall Bladder. This article explains how the planets have a connection with our human body.

ABSTRACT

To ensure that newly formulated food (A1, A2 and A3) based on local products and commercial love (AT), fed to quail reared in captivity for 9 weeks, did not affect their health biologicaly, biochemical blood analysis were carried out on them at the end of the experiment. The results obtained show that the quails fed with A3 and AT foods have the highest respective crude protein levels (55.67 ± 1.70 and 55 ± 0.82 g/l). In addition, the blood Ca/P ratio which represents the index of a good metabolism of these electrolytes, between 1 and 2, is in accordance with the recommendations in force. In addition, the average urea and creatinine levels of all the quails studied are identical to those of the control quails. Similarly, in quail fed with commercial food AT, uricemia is the highest (66 ± 0.82 mg/l) in this study. Quails fed with commercial food AT have the highest cholesterol level (2.24 ± 0.04 g/l). However, quails fed with feed A3 are those with the lowest level of triglycerides (6.80 ± 0.02 g/l). Thus, consumption of the newly formulated feeds A1, A2 and A3 by quails did not affect their renal, hepatic, pancreatic and heart health like that of quails fed the commercial feed AT.

ABSTRACT

Aims: The purpose of this study was to evaluate whether there are any significant toxic effects of the widely exposed metals on different organs (gill, liver, kidney, and muscle) of some commercially important marine fish species (Herpodon nehereus, Pampus chinensis and Hilsa ilisha). Study Design: The collection of the samples was done from January to March 2018 from Bay of Bengal near Chattogram city of Bangladesh. The study is based on randomly collected samples from the intended sampling sites (Randomized Block Design) to ensure the evenness and unbiases of the collected samples. Methodology: Analyses were performed for metals such as arsenic (As), lead (Pb) and chromium (Cr) using Atomic Absorption Spectrophotometer (AAS). During each month, five individuals each with three replications from the three targeted species were collected, with a total of 135 (45*3) fishes in the study period. Results: The concentrations of the metals found in the fishes varied in the following ranges: As: 0.029-0.071 ppm, Pb: 0.008-0.083 ppm and Cr: 0.0001-0.03 ppm. The obtained result revealed the highest concentrations of all three metals were recorded in H.nehereus fish. The organ wise lead concentration was recorded highest in kidneys and gills of examined fishes with significant variation in muscles and liver. The scenario depicted quite differently in case of chromium where concentration in gills was found to be the highest with insignificant variation in accumulation in other three organs. In case of Arsenic, kidneys and livers were the most exposed two organs in comparison to significant exposure to muscles and gills. The obtained values of arsenic accumulation were considered critical for human consumption as it was higher than the recommended values stated by WHO and FAO; but the concentration of lead and chromium were found to be safe for human consumption. The values recorded for lead were increasingly approached to the safety values. Conclusion: So, crucial steps should be taken regarding safety and environmentally friendly discharge of arsenic and lead; high levels of pollution will not only affect aquatic life but will also invite socio-economic disasters.

ABSTRACT

Objective To determine the radiation dose of sensitive organs under different protective methods in lung CT scanning environment, and to explore the best protective scheme of corresponding organs. Methods Annealed thermoluminescence dose elements were placed in the stomach, liver, colon, and thyroid gland of a simulated human body model. The dose effect experiment of protective methods included non-protective group, half lead apron group, and full lead apron group. The dose effect experiment of protective thickness included 0.50 mmpb full lead apron group and 0.35 mmpb full lead apron group. The same exposure conditions of lung CT scan were used in the above experiments. Results Compared with the non-protective group, the exposure dose of the stomach, liver, colon, and thyroid gland increased significantly in the half lead apron group (P < 0.05), and the exposure dose of the thyroid gland and colon decreased significantly in the full lead apron group (P < 0.05). There were no significant differences in the exposure dose of the liver, stomach, and colon in the simulated human body model between the 0.35 mmpb full lead apron group and the 0.50 mmpb full lead apron group. Conclusion For lung CT scan, the protective measure of lead apron may not reduce the exposure dose of subjects. The protective thickness of lead apron does not necessarily have a substantial influence on the exposure dose of human body.

ABSTRACT

Objective To study the mock circulation system (MCS) which can accurately reproduce the human hemodynamic environment for in vitro test in the development of artificial organs such as ventricular assist devices (VAD) and artificial heart-lung machine. Methods A double-heart MCS including the systemic and pulmonary circulation was established, which basically covered the main physiological characteristics and functions of the cardiovascular system. The simulation of valves and arteries were proposed with a new way made of silicone material. The MCS could simulate a variety of physiological environments such as normal human body, heart failure, valvular diseases, arteriosclerosis and peripheral obstruction changes by adjusting the control system parameters or structural parameters. The sensor and control system were used to realize the real-time display, control and data preservation of pressure and flow. Results The MCS could simulate the hemodynamic environment of normal human body and a variety of diseases, which were basically consistent with the actual human condition. The new valve and artery model reduced pressure fluctuations in a much better way. The HeartCon VAD of RocketHeart was connected to the experimental platform under simulated state of heart failure, and the hemodynamic environment (aortic pressure, left atrial pressure, cardiac output, etc.) could all be recovered to the normal range. Conclusions The MCS can accurately reproduce the hemodynamic environment of body and pulmonary circulation under a variety of physiological states, and provide an effective experimental platform for the performance test and control strategy design of artificial organs such as VAD. At the same time, the simulation method of making valves and arteries with silicone material can also be further improved in MCS.

ABSTRACT

ABSTRACT Objective to analyze the organ and tissue donation advertising campaigns as biopolitical strategies. Method a qualitative research study inspired on the Post-Structuralist strand of Cultural Studies. Data collection took place between July and September 2020 and the research corpus consisted in the visual and textual components of 421 organ and tissue donation advertising pieces from institutional websites of the Ministry of Health, State Health Secretariats, State Transplantation Centers and the Brazilian Association of Organ Transplantation. The data were managed using the Ethnograph v6 program, submitted to analysis as proposed by Janice Morse and discussed following Michel Foucault's theoretical notions of Biopower and Biopolitics. Results two categories are presented. In the first, the representation of cultural and age diversity in the campaigns was discussed as a way to include all people in the donation and transplantation process; in the second, the use of heroism, altruism and empathy to mobilize the donor's "I" was addressed. Conclusion the inclusion of cultural and age diversity and the use of characteristics such as empathy, altruism and heroism in advertising campaigns are biopolitical strategies used by the Brazilian State to drive the population towards organ donation.
